provided me with more than enough information to obtain the claim form
I requested.  Serenata's detetmination that the claim form I was
looking for was not an actual form but a structure, should prove very
valuable.  Thanks serenata, journalist and expert law - I really
appreciate the effort.

I feel that the Researcher made a highly competent and conscientious
effort to provide a completely satisfactory answer,
but the answer was plagued by a perspective different than that
intended by the question.  My original question should have
indicated explicitly a retrospective view point.  The Researcher was
not justified in assuming an ongoing process nor that the witness
signed the oath with a full knowledge of what she was doing (a layman
signing in the wrong place and time).

Goood job. I was 95% sure that it wasn't legal... but I wanted to
check. I think a combination of contacting the newspapers (hoping that
1% of 1000 will say okay) and offering free ads, would help me get
started... if I'm still interested.
