Technically, Operation Restore Hope was the U.S. component of the
United Task Force (UNITAF), which was the U.N. humanitarian and
peacekeeping effort.

So I presume that you are interested in the numbers, dates, and
locations for all forces in UNITAF.
This is a lot of information; according to the U.N. document cited
above, the forces came from "Australia, Belgium, Botswana, Canada,
Egypt, France, Germany, Greece, India, Italy, Kuwait, Morocco, New
Zealand, Nigeria, Norway, Pakistan, Saudi Arabia, Sweden, Tunisia,
Turkey, United Arab Emirates, United Kingdom and Zimbabwe", as well as
the United States. You might want to raise the price of the question
in order to get an answer that will apply to this many countries,
dates, and locations (or, alternatively, you could narrow the scope of
the question).
