What exactly is a trip generation report and how can I do one for a church?

The term "trip generation report" seems to be most associated with the
Institute of Transportation Engineers (ITE), as indicated by a search
for this term on Google:

It appears that the trip generation "report", "analysis", or the like
-- ITE calls it a "trip generation study" -- that you want to do would
be based on ITE's trip generation data and guidelines.  (If you search
on Google for "trip generation", "trip generation analysis", "trip
generation studies", and so forth, ITE comes up frequently, just as
with the term "trip generation report".)

A trip generation analysis (which as suggested above, appears to be
the same as a trip generation report) is defined by one transportation
engineering manual  as: "A data collection and analysis process that
is used to estimate the number of trips that each zone will produce
and attract."  "Zones", in turn, are: "Regions within the study area
that contain homogenous land uses and can be described accurately by
only a few variables."

The ITE publication that many refer to as "Trip Generation Report" is
actually just "Trip Generation".  It is supplemented by the "Trip
Generation Handbook".  "Church" is listed as Code 560, on page 900 of
Trip Generation, Volume Two, which contains trip generation rates,
plots, and equations.

I would imagine that a transportation engineer would recommend that
you hire a professional to conduct such an analysis.  But perhaps you
can use trip generation software to do it yourself.  A program called
"Trip Generation" by Microtrans is high up in the Google results for
"trip generation", so I presume it is relatively popular.  You might
want to contact Microtrans to see if this software would be right for
a non-engineer.  (Also, you might contact ITE to see what they
recommend.)

Incidentally, I noticed the term "traffic trip generation" in the
title of your question.  A search for this term on Google indicates
that it is sometimes used, but I think that just "trip generation" is
more common.
