How could I obtain a copy of the Millward-Ginter Outdoor Attitude Inventory?

I have called Dr. Millward personally and have requested he get back
to me at his earliest convenience about how to obtain a copy of that
paper which was his unpublished doctoral dissertation. He currently
teaches at the Indiana University of Pennsylvania. See the foot note
crediting him here:

He was nice enough to call and respond back, and said he would respond
in the commenter's section here for you to read. He does have an copy
of the paper, but it IS dated 1973 and he feels some of the material
should be revised. I'll let him post his own thoughts though.
