I recently came across a small (3 inches, or so) metal bottle opener. 
The trinket is primarily a metal cutout, in the shape of a turtle,
with 2 matching red plastic cutouts attached to both the front and
back of the metal center.

The "mouth" of the turtle functions as the bottle opener.  
The "eye" of the turtle allows ample room to attach a chain.  (to
create a keychain or necklace)

I found a supplier which offers this item in the following bulk deals

consists of merely a metal cutout and equally cheap plastic, so I am
sure these keychains cost no more than a quarter to produce in bulk.
