government form for registration of a business factitious name? Have
already been issued E.I.N # with form SS-4,IRS for Tax purposes is
this all that is needed to start my business?

Registering a fictitious business name in the State of Pennsylvania is
covered by the Fictitious Names Act of 1982.  To register a name, Form
DSCB: 54-311 must be completed.  You will need Adobe Acrobat Reader to
download the form from the Pennsylvania Department of State website.
