which can then be imported into your document. If you would you be

Please provide me with the article if the total cost including the
Google fee as well as the other fee would be under $20.00. I really
would appreciate receiving this article.

Thank you,

Greetings, Kent!

This article should be available from the Haworth Press, Inc.,
Publisher of the "Clinical Supervisor" journal. Haworth has a service
which will provide single copies of the journal's articles for $12.00
each.

BEFORE ordering, I recommend that you view this page, and check with
institution for which you work, or any libraries to which you have
access, to see if you are able to obtain this article for free:
https://www.haworthpress.com/store/ETextInfo.asp?sku=J001&Name=Clinical+Supervisor%2C+The&ProductCode=Journal&PostForm=NewPage

"DOCUMENT DELIVERY SERVICE ORDER FORM:
Most orders for electronic document delivery will be filled by return
e-mail
within 1 business day.
Request single copies of articles in Haworth journals!

If you have any questions regarding your Document Delivery Service
Request, please contact us via e-mail or by phone: 1-800-HAWORTH
[429-6784] (inside US/Canada) or 607-722-5857 (outside US/Canada).

Please note that you will need Acrobat Reader to download/read PDF
documents. Acrobat Reader is available for FREE from Adobe's website.
To download a copy of the application, click <here>"
https://www.haworthpress.com/store/DDS.asp

This page allows you to order electronic copies of the article;
however, only Volumes 11-20 are listed as available online. Therefore,
you will need to use one of 3 methods to order your article:
1) call Hawarth's Document Ordering Service at 1-800-429-6784 during
business hours (M-Th 8-5, F 8-2 EST)
2) fax your order toll-free to 1-800-895-0582
3) send an e-mail to DocDelivery@haworthpress.com

For each of these methods, you will need to provide them with the
following information:
Journal Name: "Clinical Supervisor, The", ISSN: 0732-5223 (J001)
Volume: 4 (1986)
Issues: 1-2
Pages: 51-67
YOUR E-MAIL ADDRESS: So that they can send you the document in
***pdf*** format.

I would recommend that you call personally and work with them on the
phone, to be sure of getting exactly what you need. You will want to
be VERY EXPLICIT with them that you need the article in either Adobe
Acrobat .pdf (or Microsoft Word .doc or Rich Text .rtf, if they have
it available) format. Explain to them what you are trying to do with
the chart; they may be able to send you JUST the chart in either gif
or jpg format, which would make your life much, much easier.

If they cannot send you just an image file of the chart, once you have
the document in pdf format, you will need to convert it into html or
Word (rtf) format, from whence you should be able to extract the chart
you are seeking.

If you have the FULL version of Adobe Acrobat 4 or 5 (not just Acrobat
Reader), you can download a demo version of Gemini's Iceni plugin,
which will do the conversion for you:
http://www.iceni.com/downloadSet.html

The Adobe Corporation also provides conversion from pdf format to
either plain text or html. If you choose this method, you will want to
select html.

To convert a pdf file by e-mail:
http://www.adobe.com/products/acrobat/access_email.html
This page gives you the option of attaching your pdf article to your
e-mail and sending it to either
pdf2html@adobe.com or
pdf2html@sun.trace.wisc.edu
to receive back via e-mail an html version of your file.

To convert a pdf file that has been uploaded to the web, input the URL
of the article here:
http://www.adobe.com/products/acrobat/access_onlinetools.html

If you have access to a scanner, another option would be to get a
printed hardcopy of this document, scan the chart, and save it as a
.gif or .jpg, which you can then import into PowerPoint.

BEFORE Rating my Answer:

Once you have obtained the article from Haworth, and had the
opportunity to try these steps, if you have any questions or problems,
please post a Request for Clarification, and I will be glad to work
with you to get them resolved.
