Hello John,

This is the URL that you originally asked for in your question.
http://www.usembassy-mexico.gov/svisaform.html

Here is the DS-156 form.
http://www.usembassy-mexico.gov/svisaform.pdf

This is the DS-156 Nonimmigrant Visa Application.
http://www.state.gov/documents/organization/7766.pdf

The Form I-797 is a multipurpose form issued by the Immigration and
Naturalization Service known as a Notice of Action.

This notice of action is not a downloadable form as it is an approval
notice sent by the immigrant office to the applicant.

The second most common document acknowledging approval of a person’s
case is known as Form I-797, Notice of Action. This document, which
includes several security features imbedded in the paper to prevent
counterfeiting, is forwarded to an individual upon the approval of
Form I-485, Application to Register or Adjust Status.
