You are looking two different items: a 35mm slide scanner (or an
existing scanner with a slide adapter) and software for the sound and
voice.  Are you looking for a complete solution, with both, for under
$200?

computer, and also to create a CD with sound/voiceovers to go along
with it?

I do not know of a single device that will do what you want.  There
are devices that will scan slides to files.  Now, do you want to
transfer to DVD or to CD as these are two different technologies. 
There are software applications that you can use to create either CD's
or DVD's with sound, etc.

If you would like a solution that consists of a device to scan the
slides and a software application that will create CD/DVD (let me know
which one), I can help you out.  Your $200 budget may be a bit low,
but I will see what I can find.

Hi, 

The device already exists and is with you - your computer. 

The idea is that the CD you want to make has to fall within some
acceptable format, or else no one but you can read it!

So you make a (maybe powerpoint) presentation from the slides that you
scan (using a slide scanner) and then use a microphone plugged into
your computer to record your voice, and integrate this into your
presentation.
