In a 1960s storm, the American ship "President Buchanan" sailed out of Yokohama, against the harbour pilots advice. I understand that subsequently, because she was carrying passengers who were injured, as well as losing Jeeps in crates belonging to the US Army, there was a maritime inquiry.
