lots of free/cheap software to create basic surveys.  more advanced
stuff, you'd have to have custom built.

nice thing about cnet is you can sort it by the user rating, and in
this case survey gold looks decent (only 10 reviews, hard to really
tell)...might work for you at the right price.

It depends on whether you have more time or more money. If you are
like me, it more time than money. I would get a Flash Animator program
and make my own. It is very easy and you can compile the answers in
your web account or send them to an e-mail account. A good shareware
program that is very easy to learn and makes surveys and such quickly
and easily is 3D flash Animator 3.5 from Insane tools. I found it at -
