This has certainly been a challenge, not only in finding the web sites
but also doing the research while having computer problems! I had
originally started researching one of your other countries but had to
take my computer to be fixed (it's still not working right) and when I
was able to start again I saw that another researcher was working on
the answers for you. This did not leave a lot of time to research
another country, but I decided to try.
I am posting this as an answer although there are not 30 sites. The 23

4. Questionaire: "Succeeding with Health Promotion Projects"

"Model Questionnaire has been developed on the basis of the
scientific report, "Quality Indicators for Primary Preventive Programs
at the Population Level". A survey was conducted of health-promotion
projects all over the world in an attempt to find the properties that
distinguish good projects/programs. These properties were summarized
in 13 quality indicators. The Questionnaire shows how these indicators
can be applied in public-health work. The questionnaire can be used by
anyone working with or deciding upon health-promotion projects. They
may be large or small, and run within county councils, municipalities,
organizations and associations, or companies."
A subscription costs SEK 300. It gives you online access to the
Model Questionnaire, and all updates for a period of one year. All
material may be processed for your own use on your own computer. The
subscription is paid via invoice. We plan to offer continued
subscription at a reduced price.
The report was published by the Skaraborg Institute in 1992, and in
1994 the Federation of Swedish County Councils commissioned work on
the booklet "Succeeding with Health Promotion Projects". This net
version represents a further development of the printed pamphlet, and
has been prepared by Karin Berensson, Marianne Granath and Viveca

population database, Popum. With this tool enables retrieval of
information about individuals from selected cohorts. A cohort is a

charge. The license covers a period of two months and is issued after
