Thank you for allowing me an opportunity to answer your interesting
question.
Apparently youve posed a pretty challenging question here because it
wasnt easy to dredge these cases up. After reading many case
summaries, I did manage to find a single case that meets all the
criteria you established. The other three were either filed on by an
unaffected entity on behalf of the exposed persons (plant employees
and/or residents) or were filed (or attempted) directly by affected
resident(s) themselves, seeking relief for themselves and other
affected residents and plant employees, in a class action.
The most ominous case I found, and the only one I found in which the
employees filed a lawsuit themselves alleging that negligence on
behalf of their employer was the reason for their long term exposure,
was this one not only because of their claim that exposure to
tritium caused their diseases, but because of the terrifying events
leading up to the way in which these men were allegedly exposed. I
found the facts in this case absolutely astonishing mind boggling,
in fact - and while reading it I became increasingly amazed that this
frightening incident hasnt been more widely spoken of in the media.

On November 18, 1997, Connecticut Attorney General Blumenthal filed a
$1 million lawsuit against NU, alleging that it thumbed its corporate
nose at Connecticuts environmental laws. The suit contended that
Millstone dumped amounts of hazardous chemicals exceeding state and
federal limits into Long Island Sound hundreds of times between 1992
and 1996.
In this matter, class action civil actions were attempted on behalf of
several hundred thousand residents. The court denied the privilege to
file a class action suit.
