If you go to the "The Centers for Medicare & Medicaid Services"
website: http://ww.cms.hhs.gov/
and click on "Manuals" (under the "Resources" heading)on the left hand
side of the page: http://ww.cms.hhs.gov/manuals/
and then click on "2002 Program Memos" (bottom right of the screen)
you will come to a page titled "Medicare & Medicaid 2002 Program Memos
| 2002 Program Transmittals":
http://ww.cms.hhs.gov/manuals/pm_trans/2002/memos/comm_date_dsc.asp

Then scroll down the page to the date 8/28/2002 you will see "Update
of Rates and Wage Index for Ambulatory Surgical Center (ASC) Payments
Effective October 1, 2002" if you then click on "AB-02-124" (on the
left hand side of the column) it will take you the document that lists
the wage index rates used by Medicare for ambulatory surgical center
payments.
