I'd like to know the manufacturer (name), a supplier (name&address),
and the product name of a special paper with an embedded self-adhesive
label.

The paper is A4 size, and ~100g/m2, and is designed for use with a
laser printer. It is special, in that it has a single embedded
self-adhesive label of about 4cm x 9cm (~1.5"x3") towards the top
right corner of the sheet. The carrier film for the adhesive label
does *not* cover the entire rear side of the sheet, but only the area
of the label plus a margin. I have seen this paper used, e.g. for
amazon.co.uk invoices.

For clarification, I am not looking for a standard Avery address
label. They either have a whole page of small adhesive labels (I want
'normal' paper with a single embedded label covering only a small
postion of the page), or they have a single label that covers the
entire page (I just want a small one in a corner). Also, the products
I've found so far (including Avery) have the carrier film covering the
entire rear of the sheet (Again, not what I'm looking for).
