Dear Barney4922,

You may also purchase a Physician Quality Report from HealthGrades.
http://www.healthgrades.com/consumer/index.cfm?fuseaction=mod&modtype=content&modact=shop_About_Physician_Reports_Consumer&tv_hdr=sample

The report includes:
Board certification information
State and federal sanction information within the last 5 years (if
any)
Name and address of hospital affiliation(s)
Name of health plan affiliation(s)
National comparative statistics in board certification and sanction
activity about physicians in the same specialty field
Information on how to choose a physician with a checklist and guide
You may review the samples provided at the bottom of the page.
http://www.healthgrades.com/consumer/index.cfm?fuseaction=mod&modtype=content&modact=shop_About_Physician_Reports_Consumer&tv_hdr=sample

Search for the physician here:
http://www.healthgrades.com/consumer/index.cfm?fuseaction=mod&modtype=PRC&modact=search_wizard&tpe=4&tv_physsearch=name

Enter Sakopoulos and California without entering the first name and
press “See Doctors”.

The result is: Andreas G Sakopoulos, MD

To order this report Press “Add to cart”.

The first report is $9.95 and only $2.95 for each additional report.

Repeat this time entering Gheissari and California, leaving first name
in blank.

The result is: Ali Gheissari, MD

To order this report Press “Add to cart”.

When you are finished selecting your physicians, click “Checkout.”

I hope this helps.

Best Regards,
Bobbie7
