So if we agree that the chucc is named S. Biagio e S. Maria di
Constantinopoli I'm still seeking the address and telephone number and
confirmation that it is still functioning.

I have found an address for this church, as well as the correct name. 
I do not know whether the address is still good; the page appears to
be at least five years old.

I have also found the web site, address, phone number, and e-mail
address for the diocese in which the church is or was located.  I do
not speak Italian, but I have sent an e-mail in English to the
diocese, hoping to get more information.  I will let you know if I
receive a reply.

The church hlabadie-ga has mentioned is the church I have inquired
about.  So if hlabadie-ga has found the requested information about
that church, I think it should be accepted as an answer.

The address is rather easily found, but the telephone number, if there
is one, is not listed in the paginebianche.it or the paginegialle.it.
It might be necessary to call International Directory Assistance for
Italy or the curial office, for which a number is listed.

I've also found mention of a S. Maria di Constantinopoli e S. Maria
del Soccorso in the diocese of Caiazzo in Caserta province, but I have
