settings/william bircher/my  and I've tried it without the spaces
between the words also

The uri scheme you're using is for documents located on your own
computer, not those on the web. If you haven't already, go to the site
hummer posted and either open the form in your browser or download it
(either way, you will need Adobe Acrobat Reader to view it). You then
must print it out and fill it in by hand, after which you mail it (or
physically bring it yourself) to a Social Security office. They will
be more detailed instructions included with the form.
