Hello dob-ga,

Thank you for your question.

The information you are seeking can be found at the National Archives
and Records Administration.
http://www.archives.gov/index.html

The National Archives and Records Administration (NARA) is the
official repository for records of military personnel who have been
discharged from the U.S. Air Force, Army, Marine Corps, Navy and Coast
Guard.
http://www.archives.gov/research_room/obtain_copies/veterans_service_records.html

Paper copies of military service and pension records can be ordered by
mail from The National Personnel Records Center (NPRC) in St. Louis,
Missouri.

Note: Many records were destroyed by fire in the St. Louis Center in
1973.

All requests for military records must be made in writing, signed and
mailed to the address below.

National Personnel Records Center
Military Personnel Records
9700 Page Avenue
St. Louis, MO 63132-5100

Email Access:
“Requests for military personnel records or information from them
cannot be accepted by e-mail at this time. The Privacy Act of 1974 (5
U.S.C. 552a) and Department of Defense directives require a written
request, signed and dated, to access information from military
personnel records.”

National Personnel Records Center
http://www.archives.gov/facilities/mo/st_louis/military_personnel_records.html

If you are a veteran or next-of-kin of a deceased veteran, you may
create your customized request form for your military records here:
http://www.archives.gov/research_room/vetrecs/

For all others, your request is best made using a Standard Form 180.
It includes complete instructions for preparing and submitting
requests.
http://www.archives.gov/facilities/mo/st_louis/military_personnel_records/standard_form_180.html

Costs:
“Most requests for limited information are processed without cost.
However, it is possible that a fee may be charged if the researching,
processing, and photocopying become excessive. If your request
involves a service fee, you will be notified as soon as that
determination is made.”

Response Time:
“Response time varies dependent upon the complexity of your request,
the availability of records and workload. Please do not send a
follow-up request before 90 days have elapsed as it may cause further
delays.“
http://www.archives.gov/facilities/mo/st_louis/military_personnel_records/general_public.html

Requests for the issuance or replacement of military service medals,
decorations, and awards should be directed to

National Personnel Records Center
Air Force Reference Branch (NRPMF)
9700 Page Avenue
St. Louis, MO 63132-5100

The Standard Form (SF 180), Request Pertaining to Military Records, is
recommended for requesting medals and awards.
You may download the Standard Form 180 at this direct link:
http://www.archives.gov/research_room/obtain_copies/standard_form_180.pdf

You may also submit a request by fax or by letter.

“Contact the National Archives and Records Administration's
Fax-on-Demand system. The SF 180 is available as document number 2255.
NOTE: You must call the system from a fax machine (using the handset)
in order to receive documents. Voice instructions will guide you.
There is no charge for this service except for any long distance
telephone charges you may incur. The phone number to access the
fax-on-demand system is as follows: 301-837-0990.”
http://www.archives.gov/publications/fax_on_demand/fax_on_demand.html

“Send your request as a letter. Requests must contain enough
information to identify the record among the more than 70 million on
file at NPRC (MPR). Certain basic information is needed to locate
military service records. This information includes the veteran's
complete name used while in service, service number or social security
number, branch of service, and dates of service. Date and place of
birth may also be helpful, especially if the service number is not
known. If the request pertains to a record that may have been involved
in the 1973 fire, also include place of discharge, last unit of
assignment, and place of entry into the service, if known.”
http://www.archives.gov/facilities/mo/st_louis/military_personnel_records/standard_form_180.html

Generally, there is no charge for medal or award replacements. The
length of time to receive a response or your medals and awards varies
depending upon the branch of service sending the medals.
