I work in the compliance department of a stock brokerage firm.  Part
of my responsibility is to monitor our sales force from a compliance
perspective.  I’ve decided that I will create a compliance profile for
each of the sales representatives I cover.  I’m looking for a
pre-existing MS Access 97 or Filemaker Pro 5.0 template I could modify
for this purpose.

It dawned on me that what I was really attempting to do was to create
a compliance ‘grade’ for each of our representatives.  I thought that
I might be able to modify a teacher’s grade book database to
accomplish this.

The “subject’ would be whatever compliance attribute I decided to
include.   For example, suppose I wanted to profile the subject of
Business Experience.  I might use “years in the business” as a tested
attribute.  I might assign a score of 75 to anyone in the business for
less than 5 years.   Like golf, the aim here is for a low score. 
Under the ‘subject’, I might also want to assign a score to the types
of licenses the individual had with say 100 meaning no licenses. 
Again, the lower the score, the better the profile.  I’ll want to keep
this information over time … say several years.

I’ve visited the MS Access template gallery and come away with a nice
grading spreadsheet.  Unfortunately, the ‘assignments’ or attributes
are not date stamped.  Eventually, the spreadsheet would become too
large to handle but conceptually it’s headed in the right direction.

Even if the grade book notion doesn't pan out, it illustrates what I'm
looking for.  Thanks for your help.
