There were a number of unanswered details that I attempted to answer
for you in doing this research. For example, are you looking for blank
forms or forms preprinted with logo and fixed information? Is the
position of the label on the form critical, or could they be on top?
What quantity do you desire? One or more colors?
Regardless, all of these options are avialable to you.
What you are looking for is known as an Integrated Label in the
industry. Believe me, that was not esay to find, but once I located a
company in Arizona by searching ["packing slip" with adhesive label] I
learned quite a bit about the possibilties.

Al explained that any of these options are available from blank forms
to preprinted. It might be less costly if you are not as particular
about the label placement on the form, for example, if they appear at
the top rather than the bottom. But either way can be done.
I also mentioned that it is possible you really meant to say packing
slip, which is more common in my personal experience with integrated
labels and forms, but Al says that both packing slips and invoices are
common and easily done. He was most helpful and I am sure you will

Integrated labels offer the same advantages as their
predecessor--label/form combinations. But while most label/form
combinations include labels affixed to forms, with integrated labels,
the label and form are manufactured from the same paper stock. They
are created by applying adhesive to the back of the form where the
label will be placed. The adhesive is then covered with a liner patch.
The front of the paper is die cut to create the label, but the liner
remains intact. This construction offers one main advantage over
label/form combinations: Integrated labels tend to run better through
laser printers because the label and form are created from one sheet
of paper. This makes integrated labels especially receptive to bar
code printing..."
