There are many Electronic Medical Records [EMR] products available on
the market today.  Personal tastes are always very subjective, so it's
often good to get some collective opinions from others in the medical
community.
 
I've located a site that is dedicated to reviewing medical software,
by phycisians, for physicians.  They have a table that compares over
70 software products.  This should be your first stop:

I just want to keep my PERSONAL medical records

Oops!  Well, that's definitely a different question.

I've got to ask a few more questions.  You have paper-based medical
records that you want to have loaded into a piece of software, right? 
What kind of records?  Are there graphics involved?  Do you own a
scanner?
