As you probably know, all states are required to use the DMEPOS Fee
Schedules to receive Medicare reimbursement. The schedules are updated
on a quarterly basis.

allows download of the complete 2003 Medicare DMEPOS Fee Schedules for
all regions.
Some states adjust these calculations to set their own figures for fee
schedules for workers compensation. According to Wise enterprises
there are 33 states with adjusted schedules: Alabama, Arkansas,
Arizona, California, Colorado, Florida, Georgia, Hawaii, Kansas,
Kentucky, Louisiana, Maine, Massachusetts, Michigan, Minnesota,
Montana, Nebraska, Nevada, New Mexico, North Dakota, Ohio, Oklahoma,
Oregon, Pennsylvania, Rhode Island, South Carolina, South Dakota,
Texas, Utah, Vermont, Washington, West Virginia, and Wyoming.
The link mentioned in the comments will lead you to individual contact
for each of these states so I wont list them here. Many do ask for
fees or require that you write for the information. Examples are:

a printed copy of the fees is $50. On diskette the charge is $37. Both
are available in a combined package for $59.
Arkansas:
Arkansas Workers' Compensation Commission has developed a Medical Fee
Schedule which establishes schedules of maximum fees by a health
facility or health care provider for treatment, attendance, service,
device, apparatus, and medicine. The Medical Fee Schedule, maintained
by the Medical Cost Containment Division, is reviewed and updated on
an annual basis.

of Workers' Compensation claims at usual and customary rates:

There is no mention of home medical equipment fee schedules for
workers compensation. They do specify that the 2002 Official
Connecticut Practitioner Fee Schedule is available in print for
$85.00 and in ASCII format (print copy included) for $305.00. To

The workers comp code does not mention any state schedule or
adjustment to the Federal figures.

Their guide has not been updated since 1995. They do not state whether
the home medical equipment fee schedules for workers compensation
figures are specific to the state. The printed schedule can be ordered

There is no home medical equipment fee schedule listed, however the
RMLibrary site did list this state as having as DME schedule.
