I'm trying to find some report from a reliable source that describes:
what percentage of users that visit the Contact page of a small
business and/or boutique website result in that visitor calling or
emailing the business,  and what percentage of those contacts result
in a sale.

I have been looking for some kind of statistics for you and can find
percentages for click through rates (CTR); conversion rates (CR) and
even statistics on "trust" and "credibility" regarding whether or not
you have a 'contact' page; but the type of statistics you are talking
about don't seem to be available.

To help any Google Answers Researcher who may attempt to answer your
question, is it your desire that our site's visitors to go to the
"contact" page? I ask, because the 'contact' page is more often used
when there is a problem than for reasons of beginning the purchasing
sequence.

Any additional information you can provide will help any of the Google
Answers Researchers answer your question.

Serenata,

The website hasn't yet been created.  (The goal is to run it as a kind
of micro-site and charge a fee to the client business to whom
referrals are sent.)  Contact information for that business would only
be available on the Contact page, which visitors would view only after
learning about the service offered.

I need to present my client (who is a wedding videographer) with an
estimate of the likelihood that visitors interested in learning her
company's contact information will: 1) fill out a submission form or
call, and 2) ultimately what percentage of those contacts will make a
purchase.  Another way of answering the last part is: how much less
likely is it that an extremely highly-targeted web "referral" (who has
had a chance to read about the service before contacting the business)
will become a sale than a referral generated through a live
salesperson with personal contacts?

If you can only find data to answer one part of the question and not
the other, then tell me and I will break up this question into $12
pieces.

Thanks for the clarification ...

That gives any of the Google Answers Researchers a better handle on
the information you are looking for, and also changes the generally
perceived notion of a "contact" page.

I would imagine someone will be able to find the stats you are looking
for.

Perhaps I don't understand "micro site" but I don't understand what
will bring visitors to your miro site. If most of the visitors will be
in or very near the community where the videographer is located,
perhaps 1% of the visits will lead to a sale. About one in a million
for customers 3000 miles or more away is my guess.

Visitors will be brought to the site through targeted service- and
location-specific directories and links pages.  So at least 80% of
visitors to the site will already have an interest in fulfilling a
specific need.

What I need to find out is, after they learn some details about the
service on the website, like pricing information and see photos and
read about the business and its founder, what percentage will contact
the business, and what percentage of those will ultimately purchase. 
I can make a guesstimate, but I need some industry data.  I found data
for online retailers, but I need data more relevant to industries
where a phone call is made before the purchase.

I created a website for a client with a similar business who works out
of her home and does not have a receptionist.  The sales per visitors
is a pretty standard 1 per 100 visitors.  However, the thing that she
absolutely loves about the website is that it has reduced her 'dumb
questions' phone calls by well over 90%.  When people do call now,
they almost always make appointments.

Just food for thought :-)
