Question ID: 198885
Looking for names, addresses, & telephone numbers of progressive
Northern California live-in psychiatric or psychotherapeutic
institutions. Will accept other locations in California or a nearby
state, if necessary. The institution can be private or public. Cost
is an issue, but we are willing to consider any institution, depending
on its quality. Its program must be certified by the medical board in
the state where it functions. It must have licensed medical doctors on staff &
live-in licensed psychotherapists. The adult female for whom
I’m searching has combined problems. Among them: obesity, severe
depression, anxiety, & dependency on prescription medicines. She has
had considerable experience in private psychotherapy and has been
treated by a medical facility as an outpatient - both to no avail.
The program must be able to present a timeline of how long it thinks
it will take to treat this person. It must have a program in place to
integrate, w/ follow-ups, the person back into society & employment.
It must have a good reputation & reliable, credible ways to assess its
reputation. Speed is NOT an issue in getting an answer to this
question. A week - not hours or days - would be acceptable for an
answer. Specificity & as full an answer as possible to this query is
what we need.

Clarification of Question by firebird-ga on 03 May 2003 20:11 PDT
Thank you very much for the suggestion. We tried the spiritual route.
We reached a point where we believe traditional psychotherapy should
follow. To clarify our original request, I should mention the person
in question is 45 years old, bright, & competent. During her early
adult years she was employed. She was fully functional. She worked
her way up in salary & positions of responsibility. She took part in
social gatherings w/ biological siblings, extended family, & friends.
Her dire situation is due to early childhood abandonment & a
disfunctional step-family, which caught up with her psychologically
several years ago. She began to eat a great deal. Her ability to
function steadily declined. She lost a series of jobs. She has
reached a point where she seldom leaves her apartment, & she spends
the better part of her days in bed. She is supported by Social
Security Disability. We believe with proper care & therapy she can
get back on her feet. It would be good to find a program with clients
who have similar backrounds & capabilities. It would be presumptious
for us to predict how long it might take for her to be able to
re-enter society. It would be up to the program to find the optimal
time to remain in their program. At its finish, her family would
support her - financially & otherwise - & follow the program’s
guidelines to help further her path to recovery.

Embarrassed to find the proper spelling is: presumptuous. firebird-ga

Clarification of Question by firebird-ga on 04 May 2003 14:22 PDT
Dear P1212-ga My friend is not of any belief. I would characterize
her as crotchety, disorderly, disruptive, of no niche, & in need of
strong, non-religious based therapy. To further clarify . . . in the
past she attended an outpatient clinical program at Langley Porter
Psychiatric Institute in San Francisco. The program was based on
dialectical cognitive therapy. It required that she follow rules &
regulations. She warmed to the approach. Its rules & regulations
made her feel safe. Unfortunately, the Langley program was short, &
once someone goes through the cycle there is no second go 'round. Not
long after the program, she slid back to her old ways. We are now
searching for a clinical setting with such a strong framework. She
needs & responds to "strong containers". She wants to get well, &
when she feels safe her best characteristics rise to the occasion.
I'm sorry to say that for reasons having to do with her biological
mother, she is adamantly opposed to anything related to religion or
its modern, humanistic counterparts. Thank you very much again for
asking. Sincerely, firebird-ga

Hello, firebird-ga!

The type of residential treatment facilities most suited to your
friend are called "Dual Diagnosis Facilities." These facilities treat
individuals with mental illness combined with substance abuse. Don't
let the "Substance Abuse - Addiction" treatment portion of the
facility descriptions scare you off! Although many of these facilities
do have a large proportion of clients with alcohol or drug addiction
problems, they also treat patients whose main problem is mental
illness combined with substance abuse. Mental illness and addiction
(of some sort) usually go together. In your friend's case, there is an
element of addiction associated with her dependency on prescription
drugs.

The problem is that there is a preponderance of drug and alcohol
residential recovery centers. There are also a some very good
inpatient psychiatric hospitals for those who are critically
diagnosed. But there are very few treatment centers classified as
"in-between," which offer inpatient treatment for moderate mental
illness combined with prescription drug abuse.

Rehabilitation facilities merely for drug and alcohol addiction do not
fit your friend's needs. Neither do acute care mental hospitals that
do not focus on the prescription drug abuse. She will actually benefit
most from a dual diagnosis treatment modality. I have also
included a few critical care psychiatric hospitals for you to
evaluate. However, these facilities often require a doctor's referral
and I am not sure your friend needs this type of acute care treatment.

.
The price offered for an answer to your question does not allow me to
spend the time and expense necessary to make long distance phone calls
to talk to a director of each facility. Also, the extent of issues
associated with the your friend's condition need to be discussed with
the admissions director at length. Without direct conversations with
admissions or the facility director, it is hard to get definitive
answers to all aspects of your question from online searching. For
instance, whether there are residential psychiatrists on site the
"full 24 hours", the extent of follow-up treatment including
employment, and the length of the program is sometimes difficult to
discern.

Therefore, it would be most expedient if you actually contacted the
facilities for the required information that is not available online.
I realize that a direct conversation with the facility admissions
director may weed out some of the facilities . However, you may also
be given some references which are not available online.

There are 1233 listings for mental health services in California
alone. Many are geared toward children, or are primarily outpatient
services. Unfortunately, there are no website links for the
facilities. If you are interested, the listings may be found at
"Substance Abuse and Mental Health Services Administration (California
Facilities)"

Following are the best facilities I have discovered in my research!

Dual Disorders Program
http://www.sangabrielvalleymedctr.org/index.asp?catID=ps&pg=ps_behav_dual

What is a Dual Disorder?
Also known as "SAMI" (Substance Abusing Mentally Ill), a Dually
Disordered individual has a diagnosable mental illness plus one or
more clinical issues stemming from substance abuse. Frequently the
person doesn’t realize that their drug use is only making things
worse. The individual may experience medication compliance and drug
interaction problems, prescription drug addiction (especially to
benzodiazepines and opiate derivatives), illicit drug abuse or
addiction, and alcoholism.

The problem is not uncommon yet it commonly goes untreated. It is
estimated that more than 43% of patients at Chemical Dependency
Treatment facilities and over 50% of those in Psychiatric settings
are, in actuality, dually diagnosed. This population experiences
exacerbated symptoms, compromised lifestyles, frequent relapses and,
unfortunately, severe disability and death.

We welcome patients with prior treatment failures. We also welcome the
first-timer, the skeptical, and the discouraged. We hope to help you
break the pattern.

An Integrated Approach:
Mental health problems and substance dependence problems frequently
overlap. These are complex disorders that can tear individuals and
families apart. San Gabriel Valley Medical Center (SGVMC) recognizes
that recovery from these 2 disorders is most successful when the
medical, psychiatric, and chemical dependency aspects of treatment are
integrated, and provided in an extended structured program.

No more running from one specialty center to the next. The Dual
Disorder Program at SGVMC offers step-down levels of care, and a
potential for recovery program lasting up to 12 months. Levels of care
include: acute hospital stay, intensive outpatient treatment
referrals, on-going aftercare groups, MD visits, and sober living for
a limited number of residents, 12-step recovery meetings are held
on-site nightly.

The Dual Disorders Program is San Gabriel Valley’s first fully
integrated and comprehensive treatment setting. Emotional stability,
increased functioning, and sobriety or a drug-free lifestyle are
attainable. Feel hope, joy, and acceptance. A chance for a new
beginning… like never before!

Bay Recovery Centers Inc. offers a unique blend of medical, social and

For adults who do not need (or prefer an alternative to) psychiatric
hospitalization but who are unable to live independently, Sanctuary
House provides attractive surroundings with an ideal combination of
privacy and family atmosphere. The facility is an historic,
beautifully furnished late-Victorian house, which offers a warm,
home-like environment.

The Sanctuary House Program serves a maximum of twelve adults who are
experiencing emotional difficulties; the program focuses on teaching
clients independent living skills and socialization skills necessary
for successful community living. In addition, Sanctuary House provides
a comprehensive clinical program designed to help clients cope with
intra-psychic issues that interfere with an independent lifestyle.

We begin by assessing the most appropriate level of care. Treatment
plans are then developed to meet the client's specific needs and
goals. These plans include three phases: Extended Care, Continuing
Care and Transitional Care.

Dual Diagnosis Track
http://www.soberliving.com/dual_diagnosis.htm

Dual diagnosis is a term used to describe clients who have an ongoing
disease process, characterized by more than one diagnosis. Since
chemical dependency and other disorders have a habit of walking hand
in hand, it is not uncommon for a client to enter one of our programs
seeking treatment for an addiction and in the process finds that
another disorder is present. In fact, up to 70 percent of chemically
dependent individuals may suffer from undiagnosed and untreated
secondary diagnoses.

Our Dual Diagnosis Program is well suited for clients who require help
with more than one problem. In addition to treatment for chemical
dependency, we offer individual and group therapy for trauma
resolution, gambling problems, eating disorders, and more. We also
provide medication management and individual counseling for a wide
range of mental illnesses, and other personality and behavioral
disorders.

Counseling is an important part of care. Our clients receive
individual and/or group therapy for any additional diagnosis that may
accompany chemical dependency. Our medical director, a board-certified
psychiatrist, performs all initial client evaluations. Our clinical
director, who also specializes in marriage and family counseling,
coordinates our clinical program. In addition, we have on staff a
specialist who helps clients in the areas of trauma resolution and
post-traumatic stress disorder (PTSD). Our chemical dependency
counselors are certified in many areas of counseling. These include
the treatment of gambling, eating disorders, life skills and relapse
prevention. These highly skilled and qualified people act as our case
managers, and they provide an important one-on-one link with each of
our clients and their families.

Our entire clinical staff meets weekly to review the progress of every
client in our facility. We have found that a client's progress is best
determined when a multidisciplinary team approach is used. If it is
determined that a client needs assistance with any other diagnosis or
disorder, he or she is referred to the appropriate individual and
group counselors for appropriate sessions.

Sometimes medication is a necessary part of treatment, especially in
cases of dual diagnosis. Our medical director is a prescribing
psychiatrist. He performs initial evaluations and monitors each client
who requires medication.

At SLBTS, we treat a wide variety of clients and dependencies.
However, once in awhile a client comes to us with a diagnosis beyond
the scope of our care. In such instances, we can refer the client to
an expert in their required specialty outside our treatment centers.
We help these special-needs clients find qualified specialists and
even provide the transportation and supervision that may be required
to get the client to and from an outside therapist's office.

Dear umiat-ga, Thank you very much for your comprehensive answer.
Your initial comments regarding Dual Diagnosis were especially helpful
as an introduction to the myriad treatment facilities out there. If
you are not a licensed health care professional, then you are
certainly a well informed civilian. FYI - you are right about the
fact that there is a preponderance of drug & alcohol recovery centers.
Once I entered the sites of the facilities you listed, that was
clear. The facility, Sanctuary Psychiatric Centers of Santa Barbara,
about which you said, "This sounds wonderful" is wonderful! It is
appropriate for our needs. As for their site, one of its strengths is
that it offers many excellent links. What was especially appreciated
in your answer were your kind words at the end. My wife and I thought
they were kind and endearing, and we thank you very much. We consider

Having experienced some of these ailments myself, I would recommend
your friend check out the potential spiritual roots of her issues.
The spiritual is, of course, a "belief" and so it must be approached
as something requiring reason-led faith. I can say that my own
experience has proven the reality of the spiritual's implications in
psychological problems and phenomena, and that chemical or
pharmacological approaches to solving such dilemmas can and often will
be a dead end. My embracement of the spiritual roots of my issues
resulted in 100% healing to my person. In particular, the teachings
of Christ, specifically in the areas of spiritual cleansing, may be of
great benefit to your friend.

I would highly suggest checking out http://www.raphacare.com/. The
