This is my first post to Google Answers but it looks like the perfect
place to seek help.

I have a complex set of problems which I feel are related but for
which I would greatly appreciate some advice.

There's a long-standing problem and a more recent development.

***Long standing problem***

The more long standing problem is to do with masturbation. 
Masturbation is my only sexual outlet and up until a year or two ago I
was masturbating typically once every 2 weeks.  However, over the last
year or two the frequency has gradually increased up to the point now
when I'm masturbating every 4 days or so.  Now of course this
shouldn't necessarily be a problem but the desire to masturbate has
become more of a compulsion than something I actually want to do.

What happens every few days is I get what I can only describe as an
"aura".  My hands get very hot (a surprisingly disturbing symptom), I
get feverish, dizzy, headache-y and my head generally feels like it's
swimming in hormones.  My penis (especially the tip of my foreskin)
also becomes highly sensitised (though I don't actually get an
erection) and I generally become preoccupied with the fact that I need
to masturbate.  This set of sensations can be quite overwhelming at
times and the only thing which relieves it is masturbation (though,
actually, having a nap or a sleep can also alleviate it to some
extent).

However, this isn't quite the end of the story.  For as many years as
I can remember, the effect of masturbating often leaves me feeling
exhausted for sometimes 1 or even 2 days after I've masturbated(!). 
I've kind of got used to this in the past but now that my "cycle" is
much shorter I find that I'm spending my life either building up to or
recovering from masturbation.  In other words, it's become a far too
dominant aspect of my life.

Theories?  It feels to me like a hormonal problem (it's on a very
regular cycle and doesn't depend on external stimuli) but I don't know
if such a condition exists.  My only other theory is that it's
psychological and perhaps driven by stress/anxiety.

***More recent problem***

I've had a physical problem which has been with me on and off since
mid Feb 2003.  This is a pain in the groin which at various times has
been a pain down my penis, a pain in my bladder, a pain around my
sacrum and I also get a bit of sciatica down my left leg when I lie on
my back or my left side.

As I write, the best way to describe the pain in the penis is that it
feels like something is gripping my penis very hard around the base.

My theory?  It feels like some sort of neurological problem (PNE?) -
perhaps something gone awry in or around my sacrum/pudendal
nerve/cauda equina.

Causes?  

* Overly vigorous masturbation (seems unlikely to me)?

* The result of using a recently acquired excercise bike (I've read
that riding a bike can cause PNE)?

* Related to the fact that I have a kyphosis (curvature in thoracic
spine) - maybe my spinal cord is undergoing some sort of chronic
trauma?

I'm in my late 30s by the way.

Earlier this year I presented the pain problem to 3 doctors.  I was
put on various anti-biotics (the only one which alleviated the pain
was Co-amoxyclav) but infection was ruled out.  I've also had a
prostate exam and although my prostate was quite tender, the
specialist who examined me did not think there was anything wrong with
it.

I have no discharges, no blood in my urine or semen, no inflammation,
no redness or other discolouration.  In fact there is no externally
visible aspect to the problem...  I also don't have any problem
getting an erection.

I'm actually hoping to get to a genito-urinary clinic tomorrow to
present both of these problems because (a) the pain has come back
again after a period in remission and (b) I'm getting really fed up
with both of these problems(!).  But I have to say I'm not expecting
much in the way of a diagnosis.

***What am I hoping to get from Google Answers?***

* I'd like to know if anyone can suggest an underlying cause to either
or both of the above problems.

* I'd like some suggestions for reducing the frequency of my
masturbation cycles.

* I'd like some suggestions about relieving the pain in and around my
pelvis.

* I'd like to know whether anyone thinks these two problems could be
related - they feel like they are.

Hello gymnos,

It certainly sounds like you've done your homework researching this
yourself.  Not that you'll get a satisfactory answer to your question,
but have you had any ultrasounds or radiographs done so far?

I did find this article searching using Google keywords "algorithm +
pelvic pain + male" and they mention possible relief through:

"pelvic floor massage, diathermy, botulinum toxin injections, and
transcutaneous electrical nerve stimulation. Epidural blocks, ganglion
impar blocks, sacroiliac blocks, sympathetic ganglion blocks, and
Interstim Therapy

Thanks for your initial response.  

In answer to your question, no, I haven't as yet had any ultrasound or
radiographs.  I'm assuming these are not things I can ask for - I
guess a specialist would have to arrange for them to be done.

While I'm here it's worth posting the latest developments with my
problems:

I tried to see a GU specialist yesterday but, after queueing for 2
hours, I had to leave.  I'm going to go back next Friday for another
attempt.  I did also, however, have an aromatherapy massage yesterday
and that does seem to have had a positive effect - though the pain is
still there.

The other thing I'm thinking of trying is going to see a chiropractor
to see whether they are able to diagnose and/or treat the problem.

Another strange thing about yesterday was that I was in mid-"aura" (my
hands felt extremely hot and my head felt quite inebriated and
feverish) but my genitals didn't feel particularly sensitised and I
was able to let the day pass without feeling the need to masturbate...

By the way, if no-one else responds to this question I'm happy for
your response to count as the answer.  Let's give it a few more days.

First, I really don't know whether I have a solution to your problem.
But, my inner-force says I have a hint. Before I begin to tell you
about the probable long time solution, I think I should tell you about
me. I am a practitioner of an ancient art born in the Indian
subcontinent. Its called "Yoga" (or "Yog"). I don't know whether you
have heard about it. The solution to this kind of problems is NO DOUBT
"Yoga" and "Meditation". If you can find a "Yoga and meditation centre
" or a spiritual healing center around where you stay, I am sure you
will start seeing results within first month itself. You would require
to combine this with "ayurvedic" treatment and "holistic" living. I am
not sure, whether you would like doing these or not. BUT, my
experience in this field says, "YOU ARE DEFINITELY GOING TO BENEFIT".

I'm going to tell you what I think your main problem probably is.  You
are physically addicted to masturbation.  Addiction occurs when
excessive chemicals of a specific type are frequently produced by
certain nerves (or provided by drugs).  In the long run, the chemical
receptors adapt by blocking out some of these chemicals.  Basically,
this makes it much harder to get the same "fix" and makes it necessary
more often.  It can also lead to depression.  Although masturbation is
not inately addictive, you can become physically addicted to it.  All
of your symptoms sound like normal addiction symptoms.  I suggest
talking to a doctor that specializes in addiction.  And by the way,
ranjitsisa-ga is right in that physical and mental relaxation
exercises will help you.  Anyhow, I apologize for my somewhat
undetailed comment but my memory of my physiology classes are a little
bit faint.
