I have a MicroGuard Ultraviolet Light Sanitizer made by Water
Conditioning Canada L.T.D attached to my well water pump. The
blacklight burned out and is from Light Sources and the light is #G
36T6L/2P. Where do I find this and how much does it cost?
The filtration system attached to my well kills bacteria.

I had been working on your question and today I received a reply:

Ultraviolet Resources International has those lamps in stock.

They are $39.95 ea.
The lamp is a Germicidal, not BlackLight.
