Hello,

I have had you research my cousin VLADIMIR SLADON for me in the past.
I am trying to find out the city and state where he died so I can get
his death certificate.  He died in 1996 but there is no record of this
death certificate in NYC. I do not know where he died and need a
certified death certificate very soon.

Could you tell me this or do you know you coud find this out for me? 
I have not received this information from you.

Thank you for allowing me an opportunity to answer your interesting
question.

The Social Security Death Index database does indeed indicate that
VLADIMIR SLADON (born 27 Dec 1917) died on 04 Nov 1996. His address at
time of death is listed as Bronx, New York 10466. Previously you asked
for his social security number. You will find it in this database by
searching his name as I did (I cannot, by policy, list it here for
you):

This is confirmed by an entry in the NATIONAL OBITUARY ARCHIVE, which
derives its information directly from the Social Security
Administration’s records.

There is little doubt that this is the same man you are seeking, as he
is the only one listed nation wide and his date of death matches the
criteria you specified. I know you said you had already checked with
NYC but take a look at this link and see if you contacted the right
place or if this offers any alternatives:

I might also suggest that you ask them to check the last name as a
first name as it is possible that his name might have been transposed
or even misspelled. This might have resulted in an unproductive
search. I suspect that the difficulty in finding the death certificate
probably stems from a clerical error such as this or one similar.
Armed with his name, date of birth, date of death, last known address
and social security number, you should be able to PATIENTLY assist
some nice clerk at Vital Records in locating the death certificate for
you. Kindly suggest that they search based on several (if not all) of
these pieces of information. If the death certificate is not lost, it
should be on file with them.

As a last resort, you might try the New York Medical Examiner’s Office
at (212) 447-2086.

On a hunch, you might check some of the online directories and see if
you can find any of his living relatives who might also have some
information you are seeking. As an example, I queried SWITCHBOARD.COM
http://www.switchboard.com/ and found only one person with the surname
SLADON in the entire state of New York – and I’m sure you already know
who that is.

The Social Security Death Index has been known to contain errors.  It
is possible that there may be incorrect information in the record. 
Both references provided by tutuzdad-ga ulitamtely trace back to the
same source.  Unfortunately there don't appear to any other readily
accessible independent sources that confirm this.

Also the address provided by it is that of his residence at the time
of death.  It is possible that he died in a different city, state or
maybe even country from where he was resident.

One thing to do would be check for death notices and obituaries in New
York papers around his death.  Even if he didn't die in New York city
it is possible that there is a death notice or obituary in a New York
paper that mentions where he did die.

The commenter merely states the obvious, that any record could
convievably contain an error. In truth however, Mr. Sladon was 79
years old at the time of his death and at his advanced age he was
undoutedly either receiving Social Security benefits or still paying
them at that time he died. I find it hard to believe that in either
case the Social Security Administration would have an incorrect
address for him.
