Hello turge5,
Thank you for your question.
There are a number of reasons you might see this error. For example:
Anvil Express:
http://www.anvil5k.com/_Exp-disc1/0000000b.htm
"...This message usually results from a graphics driver setting where
the Hardware Acceleration is too high. The ANVIL graphics request
fails, and the default empty document cannot be opened. Try turning
the Hardware Acceleration down to either the first or second mark from
the left. This option is found in the Troubleshooting tab of the
Advanced display settings. We are looking into a fix for this.

Question:
I am running RPM for Windows NT on a Windows 2000 operating
environment. I receive the following error when opening the RPM
user-interface: Failed to create empty document.
Answer:
Our current versions of RPM, Elite and Select both run well under
Windows 2000. Logging has been updated. If you receive this error
under Windows NT, upgrade Internet Explorer to IE 5 SP2 or greater and
your Windows NT service pack to SP5.

Why do I get the error, "Failed to create empty document" when I try
to open MINITAB?
Solution
This error message means that the Windows file RICHED32.DLL is either
missing or corrupted. Please get a copy of this file from another PC
with the same Operating System as your machine (i.e. Windows 95, 98,
NT, 2000, ME, or XP). The file is usually located in the Windows >
System or System32 folder on Windows 95, 98, ME and XP machines, and
Winnt > System or System32 folder on Windows NT and 2000 machines.
Raxco Software:

When I attempt to launch PerfectDisk Personal Edition, I get a "failed
to create empty document" message and PerfectDisk exits.
A third party registry checker such has RegClean, WinDoctor, Easy
Uninstall or System Mechanic has likely removed a registry key
required for PerfectDisk Personal Edition to work. The registry key is
written correctly by the PerfectDisk Personal Edition installation
program but is incorrectly being misinterpeted by the registry
checking software as an invalid registry key and has been removed.
Resolution:
Re-install PerfectDisk Personal Edition and do not run a registry
cleaner that incorrectly attempts to fix/repair PerfectDisk's registry
keys. After you have re-installed PerfectDisk Personal Edition ,
please be sure to AutoUpdate to make sure that PerfectDisk Personal
Edition is running the latest software.

Why do I get the following error when starting the MATLAB
Editor/Debugger on Windows?
MATLAB Editor/Debugger 'Failed to create empty document'
Solution:
This can happen if the file riched32.dll is corrupt or missing from
the following directory:
C:\WINNT\system32 on Windows NT
or
C:\Windows\system on Windows 95/98
As a workaround, you can copy this file from another machine that has
Windows.
Thomas Guide:

After installing the Street Guide Digital Edition, when attempting to
run the program it shows the splash screen and freezes, or When moving
the splash screen, I see an error message stating that "An unsupported
operation has been attempted". Upon closing the message, another
dialog occurs stating "Failed to create empty document". What should
I do?
First uninstall and reinstall Street Guide Digital Edition using the
procedures here. If that does not work contact Thomas Bros. Technical
Support at TGDESUPPORT@thomas.com.
FlashGet

Failed to create empty document occurs when running FlashGet.
1.Install a new version of Microsoft's Common Control
2.Install DCOM98 for Windows98 or DCOM95 for Windows95.
So, as you can see, there are a number of solutions depending on what
program gives you this error and what oprating system you are using.
Several of these suggestions won't hurt at all to try. For example,
Internet Explorer and Outlook Express install so many system level
files that updating these alone might easily fix the problem.
Changing the video acceleration also might be the cure, and if it is,
go to the site for your video card vendor and get the latest video
drivers for your operating system.
Also, visit Microsoft Update and be sure to choose any of the critical
or recommended updates.
If none of these work, ask for clarification and provide me with more
details such as Operating System and the specific programs or type of
attachments that show this error. I assume you might be using Outlook
Express, and if so, an update of all of the Internet Explorer
components should most likely provide the cure. If the attachments are
jpg's, for example, and you have standard system settings, they would
normally open in Internet Explorer, also pointing to a need for this
update.
