I need to find out the model # and what year this slot machine was
made.  The more details the better.  I'm selling the item and can not
find the information anywhere.  I know I can purchase a Jennings book
that has all the information, but I would rather not spend money on
the book.

I'm not a reseacher, but for fun, forwarded your question & link to a
slot machine collector I know.   His reply:

"Late 50s or early 60s machine. Illegal in many states. Wouldn't be
worth more than $400."
