I currently hold a United States Coast Guard 100 ton/Master Captain's
License and need to know how to go about renewing it. (ie, the form
number, how to obtain the form, and where to mail it to, if it can be
renewed online) Renewals are
once every 5 years and if you miss your deadline (which you are NOT
notified about) you lose you license.

GENERAL LICENSE/MMD RENEWAL INFORMATION

The Coast Guard is reviewing its entire license/document renewal
process, and changes can be expected.

“The major change is that mariners are now required to be
fingerprinted "at some point in the process" at the Regional Exam
Center, according to RADM Kevin J. Eldridge, Director of the USCG's
Merchant Mariner Task Force. Applications still can be mailed in, but
the applicant will have to report to the Regional Exam Center (REC)
for fingerprinting when they pick up the documents. If the applicants
present themselves at the beginning of the process, they will be
fingerprinted then.”

“After submitting an application at the REC, the mariner's documents
will be sent to the National Maritime Center for centralized
processing and evaluation. They are then mailed back to the REC for
pickup by the mariner.”

“New security requirements for Merchant Mariner Documents:
When applying for any MMD (originals, renewals, upgrades,
endorsements, or duplicates), mariners will be required to be
fingerprinted at the regional exam center. Mariners must produce 2
forms of identification, one of which must be a picture ID.”

Renewal applications may be submitted in person or mailed to any
Regional Exam Center (REC). This cannot be done online. Here is the

I located a LICENSE AND QUALIFIED MMD RENEWAL Packet from the U.S.
Coast Guard Regional Exam Center - Honolulu

They state:

Due to new security requirements ALL merchant Mariner Document
transactions require the mariner to physically appear at one of the
Regional Exam Centers sometime during the process. This includes
Original, Renewals, Endorsements, Raise of grade, and Duplicates.  Old
Merchant Mariner Document must be surrendered prior to issuance of a
new card.
