I'm looking for software that creates online manuals.  Here is what it
needs to be able to do:

It will be an insurance company's underwriting manual, hundreds of
pages long with many different sections, outlining to the insurance
agents how to write policies for the company.

The software must be able to create and maintain the online manual.
The online manual must function like a web site with the left side
being links (with plus signs (+) on sections that you can open up). 
The right side would be pages (content).  It must be easy to navigate.
Some sections of the manual will have different content based upon the
user who signed on (different states have different rules).
The online manual should have a search function.
The online manual should allow the user to keep their own notes on
pages.

looking for, but might require some in depth customization and
possibly some programming depending on the level of sophistication

Hi -

Can you please elaborate?  I did some extensive searching and
contacting for this question.  That software should theoretically do
what you're looking for.  Please let me know how I can clarify this
question for you and/or find something else.  Why does that product
not fit your needs?

The first product, iMarkup Document Review Server , is exactly that, a
document review server.  This is used for document colaboration.  This
is not what I am looking for.  See my original request and I don't
mention document colaboration.  Will it do what I want, well, maybe if
you stretch it, but it was designed for document colaboration, not for
creating online manuals.  Look at the bullet points on the site, none
of them match what my requirements are.

The second product, Filemaker Pro, is for making database files.  The
first thing you read is "Publish Your Databases over the Web".  What
does that have to do with what I want to do?  Again, it may do what I
want, but nowhere do I see matches to what I'm looking for.

And the other two links you gave, I found easily myself before asking
the question.
I'm sorry that I'm not happy with the results, but I'm just not.  And
google does gaurntee my satisfaction right?  That's what I read before
I put my $25 on the table.

Yes, your satisfaction is guaranteed.  You won't pay for this answer
if you're not satisfied.  As I said in the original post, the answer
is not finished until you're satisfied with it.  If you're still
unsatisfied after a clarification process, then I can withdraw my
answer [please don't ask for a refund].

An "online manual" is nothing more than a collection of documents. 
Each page is a document.  Because you're looking to maintain security
and varied access levels for each area/page, this is one way to think
of it.  iMarkup provides collaborative capabilities, but annotation
and security configuration is what I was keying in on.  There's not
much out there that provides annotation capabilities.

Questions:

1) How much, if any, work are you willing to put into the back-end
[programming/customization], or must this be a ready-made product?

2) Must this product strictly meet every one of your listed criteria
above?

None.  It must be able to be used out of the box by non technical people.

Ideally yes.  At least most of them.

I might add that Adobe Acrobat can do the same thing.  I have received
PDF files that include active indexes (click on the word and it jumps
to that section).  Automatic access to the index is available on each
page, and it even jumps from one document file to another if needed.

Even better for the user, the reader is free.

P.S.1  I have absolutely no relationship to Adobe.

P.S.2  These comments are not be a researcher.  They are free, and
worth every penny you have paid for them.

Thanks, but I am looking for something that functions as a web site,
not Adobe reader.  Our manual is now in .pdf.  It takes far too long
to open up, and many people find the reader hard to navigate.
