Hi, 

I came across this webpage about a month ago and it is probably the
most complete answer to your question anyone could find. It has all
the steps, examples of all the forms, examples of time layouts. It
even answers the question in normal project timelines and rapid
development timelines, with explanations on what the differences are
for those two types.. I was very impressed. So impressed I booked
marked it to use at work.

I can't think of what else you might need for your answer, but if you

I need to find information about the different problems and
difficulties faced on a project and how they can be overcome.  What
general difficulties/problems occur during this time from inception to
implementation?
