Would you please give me the names and contact numbers of the top
surfing camps and/or schools in Hawaii.  From what I recall there's a
well known one that is primarily for girls.

I would not recommend the Nancy Emerson School of Surfing as I had an
unfortunate experience with them on my recent vacation to Maui.  My
friend and I had prebooked and prepaid a 3-day class with them before
arriving on the island.  Once on the island we tried to reschedule the
last class in order to go scuba diving.  We were given confirmation
for the time change over the phone by the school's answering service
and thought everything was ok.  We were later left a voicemail by
Nancy telling us that the answering service was not authorized to
perform time changes, that we had missed our class, and that no money
was going to be refunded to us.  It seems highly inappropriate to
penalize us for the error of the official answering service hired by
the school.  Needless to say this put a damper on an otherwise
beautiful vacation.

Some general suggestions:

1) I would suggest not prebooking/prepaying for an extended surf class
before trying it out.  It is relatively easy to get started so you
might need less classes than you think (I suspect most people can
stand on the first lesson since the beginner boards are quite large). 
You might consider booking one
and then playing it by ear once you get to the island based on how well you
are doing and what other activities you want to try.  Renting a board is a
LOT cheaper than lessons.  In Lahaina you can rent a board, rash guard, and
booties for 2 hours for $20 while a 2-hour lesson will run you at least $60+.

2) Unless you swim or paddle a lot don't get too overly bold about how long
you can go in a session.  You spend a lot of time paddling and you can get
quite a workout in a 2-hour session.

Some more comments about Nancy's:

The Nancy Emerson School is located in a small office in a office building
and over the weekend we were unable to find anyone there.  For the first lesson
we were given incorrect instructions to wait at the office and we had to ask
around to finally locate the instructor on the beach.  Nancy herself
now resides in Australia and the office in Lahaina appears to be run
by a poorly trained skeleton crew.
