She's Joanne Kathleen Rowling.

"The use of the author's initials instead of her full name was a
marketing ploy designed to make her work acceptable to boys, who
actively choose not to read books by women."

On her birth certificate she is just Joanne, without the Kathleen. But
in an interview she explained:

"Q: Is JK Rowling your real name?
JK: My real name is Joanne Rowling. My publishers wanted another
initial, so I gave myself my favourite grandmother's name as a middle
name - Kathleen."

On March 2, 2001, Rowling was awarded the medal of Officer of the
Order of the British Empire (OBE) for her "services to Children's
Literature."  The offical etiquette to address her would be "Ms Joanne
K Rowling, OBE."
