What you're looking for is called a "reverse lookup" service/software.
 Reverse lookup works just like it sounds -- allows the lookup of an
address by knowing the phone number.  There are services online that
allow you to do it, or there is software that does it.  There are
plenty of ways to do it.  Here are some examples:
