(and in the US) and who received her qualifications from a top US
dental school practice in the UK or not?

Hi smitf7,

Ok, I finally found the website which has the information you need.
According to the IFDH and given what I know of your circumstances, you
will need to apply for training in a UK School of Dental Hygiene
(minimum six months) in order to take the National Examination to
qualify to work in the UK.

1) Procedures and/or documents that are necessary to be able to work
in UK:
"...Other applicants need to apply for training at a UK School of
Dental Hygiene."

2) Is a job offer required before a Work Permit/Visa is issued?
Yes. 

3) Are hygienists qualified overseas required to sit the country's
national examination?
Yes. Training lasting a minimum of 6 months is required before sitting
the National Examination. A list of schools of Dental Hygiene in the
UK is available from the IFDH Administrator.
