Dear Waterrat,

All of the following information is my interpretation of the MIB web
page located at [ http://www.mib.com/html/request_your_record.html ] .

MIB is legally obligated to provide individuals with the contents of
their MIB files upon request. Also, note that the method for obtaining
information may differ by country. The information I'm providing below
specifically applies to US residents. If you are not a US resident,
please let me know and I'll follow-up with the information for either
Canadian or International residents.

Under normal circumstances, MIB charges $9.00 for sending you a copy
of your MIB file.

If you have recently been denied insurance or had your premium
increased as a result of the contents of your MIB file, you are
entitled to receive a copy of your MIB report for free. According to
the MIB page, "To qualify for the waiver of the service charge, you
will need to include a copy of the letter of adverse action that you
received from the MIB member insurance company with your request for
Record Search and Disclosure. Your request must be received within 60
days of the date of the adverse action letter."

Note that another web site from within MIB [
http://www.mib.com/html/disclosure_instructions.html ] also says that
"You may also be entitled to one MIB record search and disclosure at
no charge every 12 months upon request if you can certify that (1) you
are unemployed and plan to seek employment within 60 days, (2) you are
on welfare, or (3) your MIB record is inaccurate due to fraud. MIB
will, upon request, advise you as to the certification process. MIB
reserves the right to verify requests for free disclosure."

To obtain your MIB information, you must do the following:

1) Print out one of the following forms.

2) Fill in the details requested on the form.

3) Either provide your credit card details on the form, or enclose
   a check or money order for $9.00.

   If you are requesting a fee waiver, enclose a "copy of the letter
   of adverse action that you received from the MIB member insurance
   company." I would personally recommend also including a short note
   explaining that you are requesting a fee waiver.

4) Sign and mail the request to

   MIB, Inc.
   P.O. Box 105
   Essex Station
   Boston, MA 02112

   They also list the following contact information, though it is
unclear
   whether they accept disclosure requests by telephone, fax, or
e-mail.

You should receive a copy of your file within 30 days.
