Osteonecrosis of the hip is most commonly the result of Avascular
Necrosis (AN)caused by the use of corticosteroids. This study found
that AN could occur with doses of 60mg of prednisone for as little as
a month
http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?cmd=Retrieve&db=PubMed&list_uids=12189457&dopt=Abstract

and Johns Hopkins says (at this link) that doses of 20mg or above for
extended periods will cause AN

Tetracyclines seem to be able to cure the underlying so-called
'auto-immune' diseases for which corticosteroids are typically
prescribed. For example, take a look at this paper on curing
Sarcoidosis

but I am not aware that they have any efficacious action directly upon
the osteonecrosis.
