Hi rkm100-ga :)
As per my clarification above:
I've found a US website that sells a downloadable affiliate agreement
for US$19.99 (a code is emailed to you after purchase and the document
can be downloaded as a Word document or a .pdf file). You can also
preview the document before you purchase it. I've had a quick look at
it and while it does seem to contain most of the elements that you are
looking for, you will need to do some editing and tweaking yourself.
The form is described on the website in the following manner:
<<THIS FORM:
- Provides License for Linking and Logo Use
- Provides for Tracking of Purchasers and Revenue Sharing Payment
System With Exclusions for Incomplete Purchases and Payment After
Deductions for Shipping, Taxes, etc.
- Provides for Both Parties' Maintenance of Each's Site
- Provides Standard Warranties and Indemnifications
- Provides Liability Limit
The terms of this form are in favor of the Affiliate Program owner.
See preview for form details. Purchase includes one internal license
in PDF with form fields for completion of company name, address, term,
choice of law and other terms, or Word 95 with autonumbering and fully
editable.>>
and can be found at:
http://www.coollawyer.com/webfront/doclist/docinfo.php?did=23&cbid=14
The form can be previewed before purchase at:
http://www.coollawyer.com/docpreviews/23.pdf
The preview actually gives a very good introductory guideline as to
how to use the agreement properly and discusses the issues behind
clauses such as liability limits, warranties and indemnification and
payment.
As I've indicated above, you will need to do some tweaking with this
template as it is provided only as a "basic" boilerplate template and
the disclaimers on the website make it very clear that you use such
templates at your own risk.
Of further potential interest to you:
1. An article on critical affiliate contract clauses found at:
http://www.revenews.com/advice/strategy/contracts.html
2. A guide entitled "Affiliate Marketing 101 Start-Up Guide for the
Serious Netpreneur" - this must be purchased and is downloadable for
US$25.00 found at:

(the site http://www.megadox.com/documents.php/593 itself has a
variety of e-commerce/internet documents available for download, most
costing around US$25).
Kind regards

Hi rkm100-ga
Am also confirming that purchase can be via credit card:
<<Security: Upon purchase, your credit card number is immediately
transmitted to your credit card company via secure socket layering
(see Help for more information); we do not have reference to it and it
does not reside on our site or server.>>
Best of all, if you are not happy with your purchase, then, provided
you return it within 72 hours, you are entitled to a full refund:
<<Return: If you are unhappy with your purchase for any reason, you
may return it within 72 hours of purchase and receive a full refund.
Please forward your receipt to help@coollawyer.com for a refund.>>
Of special note: Once you have made your purchase, you must download
your forms within 48 hours of receipt of your order confirmation email
- after that your code will expire.
Best wishes
jem-ga :)
