I am looking for a copy of a survey that has been successfully used
with youth to solicit their feedback on what kinds of social
reacreational activities a club should have that would be of interest
to them.

clarifications:
- When you refer to "youth survey", in which region would such survey
be conducted? (naturally, the preferences of Japanese and American
youth may not be identical)
- What kind of "club" - dance club (as in a commercial enterprise) or
a social club?
