Yes, I will be using this with High School students grades 9-12.
Majority will be 9th and 10th graders taking Algebra and Geometry
courses.

The survey instrument used by the US Department of Education's
National Center for Education Statistics may meet your needs. Their
NELS survey began in 1988, and has been updated periodically.
Their are a number of different surveys given for students, dropouts,
teachers, parents, and for initial and follow-up surveys. The most
relevant survey for your needs would be, I think, the F2-Student
survey found at:
http://nces.ed.gov/surveys/nels88/questionnaires.asp
It is a 96 page questionnaire for high school students, and covers a
host of topics. The questions about math begin at question 19.
Let me know if this survey instrument meets your needs, or if you are
looking for something different.

No this doesn't meet my need. I need a post and pre test to use with
students at the beginning and end of the school year. They are taking
Algebra or Geometry courses. I heard earlier today that a Mel
Thornton at the University of Nebraska in Lincoln has develop a survey
for math students. I don't know if that is of any help. Thank you.
