Please provide names and web address of 4-5 patent law firms which are
medium sized (about 5-10 patent attorneys), are based in area close to
Patent Office (DC, VA, MD etc), have been in business for 5-10 years,
have already large number of patents issued (provide the number) and
are engaged only in patent and other Intellectual Property law
(instead of all types legal business).
An example of such firm would be Litman Law, I think.
Optionally, you may find out if the firm has a bad reputation (if you
could check).

Please clarify: You want to know exactly how many patents have been
issues to clients of the law firms in question? I may be able to
satisfy most of your other requirements, but a listing of exactly (or
even approximately) the number of patents issued as a result of work
by a particular firm may be impossible to track down.

On another note, are you looking for firms with 5-10 PARTNERS and
other staff attorneys, or for a firm which has 5-10 attorneys total on
staff?

I had omitted to put this: The firm/ attorneys should have a track
record /expertise in process or utility patents. Kindly ensure that.

I put the number 5-10 patent attorneys to indicate the size that I
consider medium sized. So, in total the firm should have 'around' that
many attorneys. It does not matter if they are partners or principals.

To find out the number of patents issued under a patent law firm since
1976, go to http://patft.uspto.gov/netahtml/search-bool.html, enter
the name of the firm under Term1 on left, select Field1 from the menu
"Attorney or Agent" on the right.
