Hi peltab,
I haven't been able to find a state agency called "TX Commission on
Environmental Control." However, there is a state of "Texas
Commission on Environmental Quality (TECQ)," which is under the
umbrella of the Texas Natural Resource Conservation Commission
(TNRCC). Could that be what you're looking for? If so, I can give
you a link to a site that lists all 1002 TECQ forms both by subject
(title) and number, though there is no number that matches the one you
posted. However, you could use your browser to search through the
list to find terms that match the form you need. If this would answer
your question, please let me know and I'll be glad to post it for you,
along with supporting links for further info.

Are you referring to form PI-7, Registration for a Permit by Rule?

I meant to include that the site I mentioned above is one from which
you can download any of these forms, which are in one of several
formats, including Word Perfect, Excel and/or PDF.
