The Kansas Department of Health and Environment (KDHE) Office of Vital
Statistics issues certified copies of death certificates. On their
website ( http://www.kdhe.state.ks.us/vital/ ), you will find all the
information necessary in order to obtain a copy of the certificate you
require. They explain the procedures on how to request the certificate
in person, by mail, fax or by telephone. In addition, payment methods
and prices are included.

The number you must call to fax a request is (785) 357-4332. You may
fax a request 24 hours a day, 7 days a week and charge it to major
credit cards.
The phone number for the KDHE Vital Statistics is
(785) 296-1400. Telephone service hours are from 9:00 am - 4:00 pm.
They provide their e-mail if you require further information.
