diving terms, and determine whether or not this has ever actually been
attempted and/or successfully executed by anyone.

I have found a nice description of the dive.

I have not found any indication that anyone has attempted it, and I
doubt that anyone in their right mind ever would. It's always
possible that it could be on an outtake of "America's Funniest (and
Most Incredibly Dangerous) Home Videos" or MTV's "Jackass". ;-)

Anyway, if the description is sufficient for an answer, please let me
know.

Hi,

As I don't think you have proven whether or not this dive has been
attempted, I'd like to keep this question open for a while to see if
anybody else can provide this information. If we get toeards the end
of the time limit for this question, then I will accept your partial
answer.
