Hi Dotw ~
I am not familiar with the software you listed, Jim Melanson's Multi
Page Form Handler, but it looks promising. His refund policy will not
allow a refund if it doesn't work for you, though. (Click on the link
below and scroll down toward the bottom of the page).

I can comment on Willmaster's Master Form V3, though, because I use it
on my own site and I know clients who use it for a multitude of
various forms (and have no problem developing the forms for usage as
they see fit).
You can take a look at the features and benefits of using Master Form
V3 here:

When I need a cgi program, I always look first at Willmaster's site to
see if they have one, because my dealings with the owner, Will
Bontrager, have always been totally satisfactory.
The versatility of Willmaster's forms, and Master Form V3 in
particular, have been remarked on by satisfied customers in such
publications as the Link Exchange Digest, Eva Rosenberg's Help Desk &
Web Review, and I-Sales (now owned by Marketing Wonk.
Besides all the things it can and/or will do which are listed on the
website, I would recommend it for the fact that you can hide the email
address (not disguise it, it is actually concealed from view from
spambots which look at websites to harvest email addresses), and that
they offer a no questions asked money back guarantee. So if you don't
like it, at least you can get your money back.

Installing and working with forms and cgi can be confusing when you
first start. The Installation instructions are easy to follow, and
there are a lot of examples you can use to help with putting your form
together. What's even better is what you can do with the information
after you've received it. That is so versatile and easy to put
together that you'll even enjoy making forms!
If you feel totally intimidated, I'd can recommend having Will to
install it for you on your servers. He installed the very first cgi
script I had to put on a client's site several years ago.
Will Bontrager enjoys a reputation for writing clean code and I can
honestly say that I have never heard anyone complain about either his
products or his service.
If you have any doubts or questions, he invites you to ask away. He's
always happy to answer inquiries.
By the way, I use and recommend his scripts in my own business. I
don't get anything from doing so, but I am just a very happy customer
and am impressed there are still people doing business who believe in
customer satisfaction.
Hope this helps and that you get your forms up and running very soon,
no matter which software you choose.

I relied on my own experience and knowledge of both cgi scripts for
forms handling and Master Form V3 in particular.
