I want a list of the 50 USA states with a yes/no indicator, whether
the state government collects municipal and/or school district EIT
(earned income tax) along with state income tax, and then returns the
local collection to the localities.  If there is a state that does
this collection, list contact info for the state office responsible
for collection and payment to localities.
