I am in the need of a simple program (executable with source code)
which more or less performs the following task outlined below:

1) a WindowsXP user either logs off or switches users
2) a simple popup is executed before shutdown with a text field and OK
button.
3) string is submitted and concatenated to a very well hidden log file
along with a time stamp.  I will pay extra if the program can
dynamically retrieve the actual date/time off of the web IFNOT use
static computer time.  (if nothing was entered in the box, then use a
string to identify that)
4) computer finishes shutdown process

Correct answer/implementation should be very simple.  If possible,
this program should not run in the background during session.  By
answering, author turns property/ownership of code and executable to
me.

This sounds like a homework assignment, lol...but it isn't.  I am a
computer science student, however I don't have the know-how on doing
something like this.
