The assignment is to ask members of a Wellness Center(health club,
like a gym at a college campus).  They teach Kinesiology and are a
health club and a gym.

So far I have too many Yes/No questions that could probably be posed
as a few questions with various answers. I want a group of questions
that show what the members think of the place and the people running
it. Also what they are getting out of it. In a sort of 10 question
format. Preferably not needing further explanation.

Here is what I'm working on to change into a more intellegent question
using a few basic facts of fitness and health.  10 questions I came up
with below.  What I'd like is help re working it.  The type of
question I want more of is Question #1.  They don't all have to be
like that.  I'd just like a few more re worked to have a question with
some choices that are more than yes or no.

If I've completely confused you please ask me to clarify.
